Enrollment

To enroll your child at Hastings Child Development Center please follow these steps below.

Program Tour

Please call and schedule a tour at 651-438-9119 and to pick up enrollment paperwork.

Registration Fee

Hastings Child Development Center has a one-time, non-refundable registration fee of $75.00 per child. This registration fee will need to be paid in the center in the form of cash, check or money order.

Enrollment Forms

Once your registration fee has been paid, please fill out our enrollment forms that is required to be on file at the center and a read a copy of our parent handbook.

A minimum of *two *Emergency Contacts and *two* authorized Transports must be listed.. completed and submitted the packet, you then will need to have your physician fill out the Health Care Summary and Immunization record. We encourage you to also fill out your ACH form for payment otherwise, additional tuition fees could be charged for late payments. All paperwork must be filled out in complete detail and returned prior to the first day of care.

Forms Needed from your Physician

Hastings Child Development Center and the Minnesota Department of Human Services Licensing Division require that all children have a current Health Care Summary and Immunization Record on file prior to the start of care. These forms must be filled out by your primary care physician and can be faxed to the center at 651-438-9112. You need not schedule an appointment for this as long as your child has had a recent well check, most clinics will process the forms and send them within 3-5 business days.
